Position Overview

We are seeking a compassionate and dedicated

Nurse Practitioner (NP)

to join our growing healthcare team in Ashland, KY. This full-time opportunity focuses on providing high-quality primary and preventive care to residents in skilled nursing and long-term care facilities.

In this role, you will have the opportunity to build meaningful relationships with patients, manage complex medical conditions, and make a lasting impact on the health and quality of life of the geriatric population.

You will work collaboratively with physicians, nursing staff, and interdisciplinary care teams to deliver comprehensive, patient-centered care in a supportive clinical environment.

Key

Responsibilities

Provide comprehensive primary, preventive, and ongoing medical care to residents of skilled nursing and long-term care facilities.

Evaluate, diagnose, and manage acute illnesses, chronic conditions, and changes in patient health status.

Develop and implement individualized treatment plans, including chronic disease management and advance care planning.

Prescribe medications, including controlled substances, in accordance with Kentucky regulations and applicable prescribing authority.

Order, interpret, and follow up on laboratory tests, diagnostic studies, and medical treatments.

Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care.

Communicate regularly with facility staff regarding patient conditions, treatment recommendations, and care plans.

Maintain accurate and timely clinical documentation using electronic medical records (EMR).

Promote preventive healthcare, patient education, and improved quality of life for residents.

Qualifications

Active, unrestricted Nurse Practitioner license in Kentucky.

Current board certification as a Nurse Practitioner.

Active DEA registration or ability to obtain one.

Clinical background in adult, family, or geriatric medicine.

Minimum of one year of clinical experience preferred.

Experience in skilled nursing, long-term care, or post-acute care settings preferred.

Strong clinical assessment, diagnostic, and patient management skills.

Comfortable managing medically complex patients with multiple chronic conditions.

Proficiency with electronic medical records.

Excellent communication, organizational, and collaborative skills.

Behavioral health or psychiatric experience is a plus.
