We are seeking a licensed
Physical Therapist (PT) or Physical Therapist Assistant (PTA)
to provide private, in-home physical therapy services in
Southampton, NY and the surrounding area .
This is a flexible,
per diem / 1099 independent contractor opportunity
ideal for a clinician looking to supplement an existing schedule while providing high-quality, individualized care in the home setting. Visits are offered on an as-needed basis, with flexibility based on clinician availability.
Clinicians provide one-on-one physical therapy services in patients’ homes. Visits are offered based on patient needs, geographic coverage, and clinician availability.
This is an
as-needed position with no fixed weekly schedule or guaranteed number of visits . Clinicians may accept available visits based on their availability.
Perform physical therapy evaluations and reassessments
Develop and update individualized plans of care
Provide skilled physical therapy treatment in the home
Address mobility, balance, gait, strength, safety, and functional independence
Educate patients, families, and caregivers
Complete timely and accurate clinical documentation
Communicate with other members of the patient’s care team as appropriate
Provide skilled physical therapy treatment consistent with the established plan of care and applicable supervision requirements
Carry out therapeutic exercise, gait, balance, mobility, strengthening, and functional activities as directed by the PT
Monitor and document patient response and progress
Communicate patient changes or concerns to the supervising PT
Educate patients and caregivers as appropriate
Complete timely and accurate clinical documentation
Current
New York State license as either a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
Graduate of an accredited PT or PTA program
Ability to work independently and professionally in patients’ homes
Reliable transportation and ability to travel to patients in
Southampton and the surrounding area

Employment:
Per diem / 1099 independent contractor

Flexible / as needed
In-home patient visits
No guaranteed minimum number of visits or hours
Potential for additional visits as patient needs grow
Compensation
$125–$200 per visit , depending on clinician type (PT or PTA), visit type, experience, and the specific needs of the assignment.
This is a
