Are you an experienced Registered Nurse with a background in
Home Health, Medicare compliance, and OASIS assessments ? We're looking for a dedicated
RN Clinical Manager
to lead clinical operations, support care teams, and ensure the delivery of high-quality patient care.
Oversee and coordinate direct patient care services.
Review patient assessments, reassessments, and Plans of Care (POCs).
Ensure compliance with Medicare Home Health regulations and OASIS documentation requirements.
Coordinate care with clinicians, physicians, patients, and caregivers.
Supervise, mentor, and evaluate clinical staff.
Participate in recruitment, onboarding, training, and staff development.
Review Start of Care (SOC), Resumption of Care (ROC), and recertification documentation.
Promote clinical excellence, regulatory compliance, and positive patient outcomes.
Required Qualifications:
✅ Graduate of an accredited School of Nursing
✅ Active Registered Nurse (RN) license
✅ Current CPR certification
✅ At least 1 year of management experience in home care, hospice, or a similar healthcare setting
✅
Required: Medicare Home Health experience, including hands-on OASIS assessments, documentation, and compliance
✅ Valid driver's license, auto insurance, and reliable transportation
