Job DescriptionSummary
Responsible for coordinating and overseeing patient care delivered by the clinical team. Serves as a clinical resource to support staff and promote professional, independent practice. Demonstrates a strong commitment to excellent customer service for patients, families, and internal teams. Delivers patient- and family-centered care, supporting the mission to improve health and quality of life. Ensures age-appropriate care and adherence to patient safety and quality standards. Maintains professional competency through continuing education and engagement in professional development activities. Performs other duties as assigned. Fulltime position | 80 hours per two weeks | Salaried position | no on-call shifts Typically, Monday through Friday, 8:00AM to 4:30PM Working from Redwood Falls and Willmar Pay and Benefits
Starting salary begins at $96,283 annually and increases with experience Full salary range: $96,283 - $144,414 annual salary

Graduate from an RN program. Bachelors or Master's degree preferred but not required. Current Minnesota Registered Nurse License. Valid Minnesota Driver's License. Minimum of one year of experience in Hospice and knowledge of regulatory guidelines.
Strong communication skills both orally and written essential. Knowledgeable in computer-based data entry.
