Pay Range: $38/hr - $42/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

Conduct in-person member home visits for assessments and care coordination

Review medical records and prepare for visits to ensure comprehensive evaluations

Submit assessments within 24 hours and complete documentation on time

Coordinate member care and facilitate transitions to permanent care managers/service coordinators

Complete required training and assigned tasks promptly

Key Requirements and Technology

Experience

Active RN license in

McAllen Area, TX

3+ years of nursing experience

2+ years of case management experience

Experience in pediatric care is preferred

Strong clinical judgment, accountability, and prioritization skills
